Elizabeth Dunkley PT, DPT

“Becoming the Director of Rehabilitation at Warren Center is a dream come true.” That’s how Elizabeth started her career here, and that’s the attitude she brings to our therapy suite every day. Pretty great, huh? But Elizabeth isn’t new to the Centers Health Care family. After earning her doctorate in Physical Therapy in 2010 she moved to Queensbury and was most recently at the helm of the outstanding therapy team at our sister facility in Granville. For Elizabeth, therapy is truly a “family affair”– her husband Ben is the Director of Physical Therapy at the nearby Washington Center! “We have such a wonderful and knowledgeable crew of therapists here. It’s an honor to be part of this team.” And it’s an honor to have Elizabeth leading the way.

Robin Chiaravalle MSW

Director of Social Services

Robin is an “almost native” of Warren County. She was born in NYC but grew up in the Lake George area where she calls her home. With a Master’s Degree in Social Work and more than 20 years of experience, Robin is well suited to meet the needs of our residents. Her entire career has been devoted to providing for long term care residents and their families, and she can often be found providing counseling to adults, children and families. “Providing support and advocating for vulnerable populations isn’t something that you can limit to the resident. Moving in to a long term care facility is a family decision so we embrace the whole group. We’re a family here at Warren Center, and a big one at that! And one of the things that keeps that growing are the new residents who move in and out for a period of rehabilitation. I love working with them, knowing that we’re playing an instrumental role in their recovery and helping them transition from the hospital to Warren Center and then back to their own homes. We can help with all sorts of community resources that can be helpful after their stay with us is over, and it helps keep us connected. Once a family member, always a family member.”

For the absolute highest quality of care, Warren Center staffs medical providers on each unit who stay in constant communication with leadership throughout the day. Team collaboration is our forte, and this commitment helps us maintain an unbeatable continuum of care.

Our team also works hard to keep therapies as consistent as possible, tracking all communications through an advanced electronic medical record system. This ensures immediate transmission of information across the entire care network.
